iOR Partners, a provider of ophthalmic office-based surgery (OBS) suites, announced the launch of a collaboration model that unites ophthalmologists and optometrists in what it is calling a first-of-its-kind partnership. This approach is designed to establishe a streamlined continuum of care that benefits patients while enabling ophthalmologists to maintain autonomy and financial stability amid declining reimbursements.

iOR Partners’ new collaboration model is designed to help physicians pivot toward a primarily cash-based system—creating financial resilience while preserving their ability to serve all patients, including those on Medicare and Medicaid.

“The reality is clear—reimbursements are falling and corporate giants are moving into the surgical space,” said Tony Burns, Chief Strategy Officer of iOR Partners. “To preserve independence and continue providing high-quality care, we must shift away from third-party dependency. The MD/OD OBS collaboration model empowers surgeons to regain control of their practice, serve patients on their own terms, and thrive in the changing market.”

Legacy Eyecare and Kugler Vision, both based in Omaha, Nebraska, are the first to pioneer this MD/OD OBS model. According to iOR, the collaboration dismantles long-standing barriers between referring ODs and surgeons, fostering cohesive partnerships that enhance both patient care and practice growth.

Benefits include:

  • For ODs: New revenue streams, improved patient retention, and a more unified care delivery model

  • For Surgeons: Greater surgical volume, a reliable referral network, and full clinical control

  • For Patients: A seamless, patient-centered experience without the fragmentation typical of referral-based systems
